Goals: Activate and engage patients, improve outcomes, and increase utilization of telehealth services by 20 percent.

LifeAlytics Use Case: The LifeAlytics platform would be applied to a cohort of existing patients that are struggling with prescription adherence and follow-up appointments due to low availability of PCP appointments. Enrollees would be given free access to the platform, which would include a direct connection to the health system’s providers through a virtual care mechanism.

By switching away from difficult in-person visits to virtual visits, patients using the platform are more likely to complete the recommended follow-up visit with their physician and can share ongoing updates about their health status in-between visits. Through targeted content, care teams can serve prescription reminders to patients with the goal of increasing filled prescriptions and improved levels of adherence.

Member Risk Management

Who: Third-party administrator for self-funded insurance plans that has built a suite of programs and vendors to drive health risk management in a multi-employer environment.

Goals: Identify individuals that are at risk for a catastrophic health event by integrating claims data, consumer data, predictive modeling, and wellness data. Drive up utilization of the programs that can mitigate this risk by 15 percent.

LifeAlytics Use Case: Various vendors are integrated into the LifeAlytics wellness platform so that all consumer solutions are in a single place. The various data platforms are integrated via API to create a Unified Risk Profile. Care protocols and engagement are driven to the consumer through a multi-tiered incentive program that includes premium differential management.

Who: A Large employer that wishes to enter into a provider direct contract with their local hospital and encourage their employees to leverage that contract to better manage their health.

Goals: Drive employees to use the health services available to them for health management, prevent the onset of chronic disease, facilitate the management of acute patients, and improve the access to care at the local facility through virtual care solutions. The company would also like to leverage incentives and premium differentials for employees while maintaining a HIPAA compliant environment.

LifeAlytics Use Case: LifeAlytics integrates with the existing wellness vendor, disease management solutions at the local hospital, and the hospitals community-facing health management solutions. Various health risk assessment data is compiled and the population risk stratified. Based on individual risk and need criteria, employees are provided with customized recommendations from the LifeAlytics engine that are pre-programmed by the health and risk management entities. Employees are given the opportunity to log completion of recommended activities, all which may be validated by the hospital or wellness vendor. Actions, compliance, and outcomes are rewarded simultaneously via premium differentials, contributions to a Health Savings Account, and a community-based reward program. The LifeAlytics engine provides the calculations on input measurements and recommendations, the employer receives aggregate reporting on risks, activities, and outcomes that meet HIPAA requirements.

Outcomes Driven Health Plan

Who: An insurance carrier has partnered with a local hospital system to provide a series of health plan options to employers that resemble narrow network plans combined with value-driven metrics.

Goals: Create a competitively priced program that will generate new membership. Drive utilization of partner hospital services. Engage members preventatively. Identify risk cohorts and drive compliance solutions to enable the hospital to maximize its reimbursement structure. Develop an ability to offer premium differentials based on care compliance.

LifeAlytics Use Case: The existing population health, medical home, and community health programs are fully integrated into a unique LifeAlytics program that provides the consumers with a single point of care access. Employers who are not ready to move to a new health plan are provided with the unique wellness and health program of LifeAlytics that can serve as a corporate wellness initiative at highly reduced cost. This serves the point of embedding the health services in the employer prior to joining the health insurance plan. Employers are then converted to the health plan at the enrollment date. The employees have already engaged their new primary care physician virtually, are aware of all of the hospital’s services, and know of the specialists that can help them with unique needs. Quality metrics are identified, risk inputs created, and care protocols programmed into the LifeAlytics system to meet the needs of patients and generate maximum reimbursement.

Community Health Initiative

Who: A small rural county has received a negative health needs assessment and wishes to improve the access to care and outcomes for its local population. They have banded together with their local municipalities, local health providers, local government services, and various non-profits, yet there are still gaps in access to the required care.

Goals: Develop a community-based mobile application that ties together all of the local resources. Use this application to not only promote health events, but also generate ongoing updates and reporting on all the metrics in the needs assessment while identifying other needs and gaps in care. Create incentive programs funded by local non-profits that compensate local residents for signing up for the program, taking health risk assessments, and acting on recommendations generated outside of the local provider group.

LifeAlytics Use Case: LifeAlytics is implemented in a way to consolidate all community resources into a single application while also giving the individuals access to the services of their existing preferred care provider. The various virtual care resources of the local providers are added to the platform, and third-party virtual solutions from larger metro communities are added to enhance the continuation of care while still driving local medical home utilization. HIPAA, privacy, and referral standards are maintained as risk inputs, and recommendation outputs were pre-programmed into the LifeAlytics recommendation engine and work autonomously from the providers. The community-based health fund is also managed by the LifeAlytics incentive engine separately from all care providers.

Ongoing Consumer Insights

Who: An analytics firm provides aggregated data sets to its clients for marketing campaign guidance and prospect identification. The data is aggregated on a regular basis but the firm’s clients cannot digest and act on it quickly enough, resulting in lost sales and lost opportunities for the firm’s clients.

The wellness tracker can be branded uniquely for each one of the firm’s clients, giving them a consumer-facing application at a fraction of the cost of custom development. The LifeAlytics platform provides the analytics company with the unique ability to provide real-time consumer insights, updates, and customized outputs based on identified consumer attributes. The app can also be connected to each client’s inside sales via a virtual consult delivery or API integration to a CRM, providing an increase in conversion rates for the client.
